Step 2- Once you’ve selected your wellness provider, complete the questionnaire then you may request an appointment from our calendar below for your consultation.
**Pre-Assessment: Services begin by scheduling a free consulation call. We will answer any questions you might have and determine if we’re a good fit.
Step 3 - Revive Counseling & Wellness is a paperless practice. After scheduling your appointment, our intake coordinator will email you our online intake form. Please fill out the intake form at least 48 hours before your first appointment on any tablet. Smartphone or computer.
** Please note, the form take about 5-10 minutes to complete, as it included questions that help your clinician get to know you. Waiting to complete the intake form may cut into your initial session time, we strongly advise doing so ahead of time.

OUR RATES, INSURANCE, & ACCEPTED
PAYMENT METHODS
$120 per 50 minute individual session
$140 per 50 minute couples session
$180 per 90 minute couples session
Insurance
We do not accept insurance at this time.
Payment
We accept cash, check and all major credit cards as forms of payment. We also accept payments via HSA, and FSA.
CANCELLATION POLICY
We prioritize our clients by holding space for each person we see. When clients cancel an appointment, our clinicians offer that space to other clients who need an appointment that week. When clients cancel within 24-hours or no show, other clients miss out on an opportunity to be seen.
If you would like to cancel an appointment, we ask that you do so using the client portal access at least 24-hour notice. Anyone canceling an appointment with less than a 24-hour notice will be charged the full session fee.
We will charge the credit card on file for no shows and cancellations within 24-hours of scheduled session.
